CE Reprint Request

If you have received CEs through Behavioral Tech in the past, and you want to request a reprint of your missing or lost document, please fill out the form below, and pay the non-refundable CE copy request fee (shows as "trial" in the credit card form).

The administrative fee will be charged ONE TIME when your CE is located (shows as "monthly", but is only one time). Your certificate will be sent to your email address.

Administrative fees are $195 for CEs before the year 2016 and $45 for 2017 to the present year.
